Waist-to-height ratio is your waist circumference divided by your height, and it is one of the few longevity-relevant numbers you can obtain at home in a minute with nothing but a tape measure — no clinic, laboratory or scanner is involved. Both go in the same units, so centimeters or inches work equally well. The whole difficulty is in the tape: height barely moves in adulthood, so almost all of the error, and all of the change, sits in the waist.
What you need
- A flexible, non-stretch tape measure. An old softened cloth tape reads short; a metal builder’s tape will not follow the body.
- Your height without shoes, measured against a wall, not recalled from a driver’s license.
- A mirror or a second person, to confirm the tape is level all the way around.
- Bare skin or one thin layer. Waistbands, belts and sweaters all add centimeters.
- A fixed routine: same time of day, ideally morning before eating, and the same landmark every time.
How the measurement works
Stand upright, feet hip-width apart, weight even, arms relaxed. Place the tape horizontally around the bare abdomen, snug enough to stay put but not tight enough to compress the skin. Read it at the end of a normal breath out, without pulling your stomach in. Measure two or three times and average; repeats by the same person commonly differ by about a centimeter.
Where the tape sits is the part people get wrong, and protocols disagree. The World Health Organization protocol uses the midpoint between the lowest rib and the top of the hip bone; United States survey protocols use the top of the hip bone itself; others use the navel. Each gives a slightly different circumference, so what matters is not which you pick but that you never switch: changing landmark can imitate months of progress, or erase it.
Measure height with heels together against the wall, back straight, eyes level, at the end of a deep breath in. Then divide waist by height. The Waist-to-height ratio (WtHR) calculator does that arithmetic, categorizes the result by sex, and works backwards to the waist circumference that would put you inside the healthy band at your height.
How to read the result
The bands below follow UK NICE guidance for adults. Other bodies publish slightly different boundaries, some of them sex-specific, so read your ratio against one scheme at a time.

The plain version is the 0.5 rule: keep your waist under half your height. It applies to men and women alike, which is why it travels better between populations than an absolute waist cut-off. It says nothing about your total body size, and that is what Body Mass Index (BMI) is for. Given the measurement noise, treat a shift of one or two hundredths as nothing; watch the direction over months.
What it does not tell you
A tape cannot see inside you. It cannot separate deep abdominal fat from fat directly under the skin, and the deep depot carries most of the metabolic risk; only imaging tells them apart. Nor can it separate fat from muscle, bloating or fluid, so a strong abdomen and a distended one can read the same. Adding the hips, as WHR (waist-to-hip ratio) does, describes fat distribution but still does not look beneath the skin.
The adult bands do not apply in pregnancy, and children are read against age- and sex-specific references. At a very high body mass index it adds little, since nearly everyone is already above the threshold. Risk at a given waist also varies by ancestry, with lower thresholds recommended for South and East Asian populations. The evidence linking the ratio to disease and death is observational: no trial has randomized anyone to a waist-to-height target. It is a marker worth watching, not a treatment goal, and says nothing about your blood pressure, glucose, lipids or liver.
