Visceral fat is the fat stored deep inside the abdominal cavity, packed around the liver, pancreas, and intestines, rather than in the layer just under the skin. Subcutaneous fat is what you can pinch; visceral fat sits behind the abdominal wall and cannot be pinched at all. The distinction matters because the two tissues behave differently, and two people at the same weight and body mass index can carry very different amounts of it.
What it measures
Physically, it is adipose tissue in the omentum and mesentery plus the fat pads directly around the internal organs. The reference method is a CT or MRI cross-section through the abdomen, usually near the level of the navel, which reports visceral adipose tissue as an area in square centimeters. A DEXA scan estimates it from a defined android region, and consumer bioimpedance scales output a unit-free “visceral fat rating” that is manufacturer-specific and not interchangeable between devices.
Almost nobody gets imaged for this in routine care. The everyday proxies are Waist circumference, measured at the top of the hip bone at the end of a normal breath out, and waist divided by height. Why it matters for longevity
Visceral fat is not inert storage. It is metabolically active tissue that drains directly into the liver through the portal vein and releases free fatty acids and inflammatory signaling molecules while producing less adiponectin. That pattern is closely tied to Insulin resistance, fat accumulation in the liver, and chronic low-grade inflammation.
Across large cohorts, higher visceral fat is associated with type 2 diabetes, hypertension, cardiovascular disease, several cancers, and higher all-cause mortality, and the association persists after adjusting for body mass index. That is why a normal weight does not clear you. This evidence is largely observational, so it shows association rather than proof of cause; what makes it persuasive is its consistency across populations and a mechanism that is well understood.
What changes it
A sustained energy deficit works, and visceral fat often falls early and proportionally faster than subcutaneous fat during weight loss. Regular aerobic exercise reduces it in trials even when body weight barely moves, so the scale can understate progress. Resistance training keeps the loss from coming out of muscle. Nothing targets the abdomen selectively — crunches build muscle under the fat and do not remove it.
Heavy alcohol intake and sugar-sweetened drinks push fat toward the liver and abdomen. Short or disrupted sleep worsens insulin sensitivity and appetite regulation. Menopause shifts storage toward the abdomen as estrogen falls, which is why the waist can grow while weight stays flat. GLP-1 receptor agonists and bariatric surgery reduce visceral fat substantially, but they are medical treatments with trade-offs and belong in a conversation with a clinician. Age and genetics set part of your baseline, and those you do not control.
