Metabolic syndrome is a cluster of five cardiometabolic risk factors – a large waist, high triglycerides, low HDL cholesterol, raised blood pressure and raised fasting glucose – diagnosed when any three of the five are present at the same time. It has no symptoms of its own. It is a label for a pattern that shows up on a routine blood panel, a cuff and a tape measure, and it says fuel handling has drifted long before diabetes or heart disease is diagnosed. In US survey data it is common – estimates run near one in three adults – and prevalence climbs steeply with age.
What it measures
Nothing exotic is involved: a tape measure around the waist, a blood pressure cuff, and a fasting blood draw reporting triglycerides, HDL cholesterol and glucose. Treatment counts as a positive criterion, so medication for blood pressure, triglycerides or glucose scores that item as met.
The five items cluster because they share a mechanism. Excess Visceral fat – the fat packed around the abdominal organs rather than under the skin – releases fatty acids and inflammatory signals that leave the liver and muscle handling insulin poorly. That state of Insulin resistance pushes glucose and triglyceride production up, drags HDL down and raises blood pressure. The waist measurement is a cheap proxy for the fat you cannot see.
Typical values
The criteria below are the harmonized set agreed by the major cardiology, diabetes and public health bodies; any three make the diagnosis.

The waist cut-off is the one that genuinely moves. Lower thresholds are recommended for South Asian, East Asian and several other populations, in whom metabolic trouble appears at a smaller waist, and some criteria set a lower bar for everyone. Why it matters for longevity
In pooled analyses of observational cohorts, people who meet the criteria have roughly twice the risk of cardiovascular events and cardiovascular death, and a several-fold higher risk of developing type 2 diabetes. It is also associated with fatty liver disease, chronic kidney disease, sleep apnea and several cancers, and it rarely arrives alone.
Two caveats. This is observational evidence: the syndrome flags risk rather than proving causation. And some researchers argue the package adds little beyond its parts, since each of the five is a risk factor on its own. Its practical value is as a prompt – three items ticked means the problem is systemic, and treating one number while ignoring the rest misses the point.
What changes it
Almost all of it is modifiable, which is unusual among aging markers. Losing excess weight is the strongest lever: in the Diabetes Prevention Program, a lifestyle program targeting 7 percent weight loss and 150 minutes of activity a week cut progression from prediabetes to type 2 diabetes by 58 percent, outperforming metformin at 31 percent. Losing 5 to 10 percent tends to improve all five components at once.
Beyond weight, the levers that show up in trials are a Mediterranean-style diet, less refined sugar and alcohol, aerobic training plus strength work, and enough sleep, since short sleep worsens glucose handling and blood pressure. Where that is not enough, medication treats the individual components, and GLP-1 drugs have moved both weight and cardiovascular risk in obesity. You cannot change your genes, your age, or for women the shift in fat distribution after menopause. Those set the starting point, not the outcome.
