Waist-to-height ratio (WHtR)

Waist-to-height ratio (WHtR) is your waist circumference divided by your height, both measured in the same units, and it is used as a simple index of how much fat you carry around the abdomen. Someone with an 84 cm waist and a height of 175 cm has a ratio of 0.48. The rule of thumb is easy: keep your waist to less than half your height. It flags central fat in people whose weight looks unremarkable, and costs nothing beyond a tape measure. The fuller reference entry is Waist-to-height ratio (WHtR).

What it measures

The calculation has two inputs. Measure height without shoes. Measure the waist standing, tape against skin or thin clothing, level all the way around, snug but not compressing, at the end of a normal breath out. Divide waist by height; centimeters or inches both work, since the units cancel.

Tape placement is the part people get wrong. The World Health Organization protocol uses the midpoint between the lowest rib and the top of the hip bone; other protocols use the navel. The numbers differ slightly, so pick one landmark and reuse it every time. What you get is an estimate of central adiposity, not a scan: it cannot separate deep abdominal fat from fat under the skin, or fat from muscle.

Typical values

The bands below follow the boundaries used in UK NICE guidance for adults. Other organizations publish slightly different cut-offs, and the ratio is not read the same way in children, in pregnancy, or at a body mass index of 35 or above.

The 0.5 boundary applies to men and women alike, whereas absolute waist cut-offs are set separately by sex and population. Repeat measurements by the same person commonly differ by about a centimeter, so a small move is noise rather than progress.

Why it matters for longevity

Where fat sits matters more than what you weigh. Fat stored inside the abdomen, known as Visceral fat, is metabolically active and is associated in observational studies with insulin resistance, type 2 diabetes, high blood pressure, unfavorable blood lipids, fatty liver and cardiovascular disease. The waist-to-height ratio is a cheap stand-in for that depot.

In pooled analyses of prospective cohorts, the ratio is associated with cardiometabolic disease and all-cause mortality at least as strongly as BMI — body mass index, and in many datasets more strongly. Its advantage is catching the people BMI misses: someone in the normal weight range who still carries fat centrally. This is association from observational data. No trial has randomized people to a waist-to-height target, so treat the ratio as a risk marker to watch, not a goal in itself.

What changes it

  • Only the waist, in practice. Height is fixed in adulthood, so every change you can make runs through the waist.
  • An energy deficit you can sustain. Abdominal fat shrinks with overall fat loss, and the dietary pattern matters less than whether you keep it up. No food empties one fat depot selectively.
  • Alcohol and sugar-sweetened drinks. Both are associated with central fat and are easy calories to remove.
  • Aerobic and resistance training. Exercise trials show reductions in waist circumference, sometimes with little change on the scale, because muscle is gained as fat is lost. Sit-ups do not reduce abdominal fat.
  • Sleep, stress and medication. Short sleep and chronic stress are associated with a larger waist; GLP-1 based weight-loss drugs reduce waist circumference along with body weight.
  • Outside your control. Genetics set where you store fat, and hormonal change – abdominal storage rises after menopause – moves the ratio without any change in habits.
