Skeletal muscle mass (SMM)

Skeletal muscle mass (SMM) is the total weight of the voluntary muscle attached to your skeleton — the tissue you move, lift and stand with. It is one compartment of body composition, alongside fat, bone and organs, and in most healthy adults the largest tissue in the body. It is also the compartment that shrinks with age, illness and long stretches of sitting still, while fat mass holds steady or grows.

What it measures

SMM is always an estimate: no routine method weighs muscle, each detects something else and converts it.

  • DXA. Two X-ray energies separate bone, fat and lean soft tissue. It reports lean mass rather than muscle as such, and is the usual clinical reference.
  • Bioelectrical impedance (BIA). A small current passes through the body; muscle conducts it well because it holds water, fat does not. A device-specific equation turns that into kilograms — cheap and fast, but sensitive to hydration and a recent meal.
  • MRI and CT. These image muscle directly and measure its cross-sectional area — the most accurate option, the least available.

Clinical work usually narrows this to appendicular muscle, the muscle in the arms and legs. Since a taller body holds more of it regardless of health, the kilograms are divided by height squared to give SMI — Skeletal Muscle Index, the form the published thresholds use.

Typical values

There is no single agreed normal range — only thresholds for what counts as low, and they differ by device, by sex and by the group that wrote them. Asian consensus criteria set several lower than European ones. Two bioimpedance scales will also disagree on the same person, so follow your trend on one device.

Why it matters for longevity

Muscle is where most glucose goes after a meal, so less of it is associated with poorer insulin sensitivity. It is also a reserve: older adults with low muscle mass are more likely to fall, fracture a hip and lose independent living. When the loss becomes severe enough to diagnose, it is called Sarcopenia.

One caveat runs through the evidence. In cohort studies, muscle strength predicts all-cause mortality more tightly than muscle size does, which is why diagnostic frameworks test grip strength first and measure the tissue second. Skeletal muscle mass (SMM) marks what a body can do rather than guaranteeing it. The link to survival is observational, and adding kilograms of muscle has not been shown in trials to add years.

What changes it

Progressive resistance training is the lever with real trial evidence behind it. Controlled trials in older adults, including people in their eighties, increased muscle size and strength when the load rose over time — two or three sessions a week, large muscle groups, more weight as it gets easier. Walking protects the heart but does little for muscle mass.

Protein supports that training rather than replacing it. Guidelines for older adults commonly suggest more than the standard adult reference intake of 0.8 g per kilogram per day, spread across meals rather than loaded into dinner. Calories matter as much: losing weight fast takes muscle along with fat. Creatine monohydrate alongside training has modest supporting evidence.

Losses come faster than gains. A week in a hospital bed can strip muscle that takes months to rebuild, so moving early after illness or surgery matters. Uncontrolled diabetes and chronic inflammation speed it up. Age, genetics and falling sex hormones set the background rate you do not control.
