Body composition is the breakdown of your body weight into fat mass and fat-free (lean) mass, which includes muscle, bone, organs and water. It answers the question a bathroom scale cannot: not how much you weigh, but what that weight is made of. Two people of the same height and weight can carry very different amounts of muscle and fat, and the difference shows up in strength, metabolic health and how well they age. That is why health is increasingly described in terms of Body composition rather than weight alone.
What it measures
The simplest model splits the body into two compartments: fat mass and everything else. More detailed models divide lean mass into skeletal muscle, bone mineral and body water, and separate fat under the skin from fat packed around the abdominal organs.
Several methods estimate that split, and they do not all agree. A DEXA scan uses low-dose X-rays to report fat, lean tissue and bone region by region, and serves as the practical reference standard. Bioelectrical impedance devices, including smart scales, send a small current through the body and infer fat from resistance; they are convenient but sensitive to hydration, food and time of day. Skinfold calipers depend on the skill of whoever holds them. MRI and CT image abdominal fat directly, mostly in research. A tape measure around the waist is crude, free, and still informative.
Because the methods disagree with each other, the trend matters more than the absolute number. Track yourself with the same device, at the same time of day, under similar conditions.
Typical values
Body fat percentage is the most commonly quoted output. The bands below are the widely cited fitness categories, not diagnostic cut-offs; healthy ranges shift with age, and different organizations and devices publish different boundaries.

Lean mass is usually reported as an index, dividing fat-free or appendicular muscle mass by height squared so that tall and short people can be compared. Thresholds for low muscle mass vary by sex, reference population and equipment, so use the reference range your lab or device supplies rather than one from elsewhere.
Why it matters for longevity
Much of the risk usually attributed to body weight tracks its composition rather than its total. Fat stored around the abdominal organs, known as Visceral fat, is metabolically active and is more strongly associated with insulin resistance, type 2 diabetes and cardiovascular disease than fat stored under the skin. Two people with the same BMI can therefore face very different risks.
The lean side matters just as much. Age-related loss of muscle mass and strength, called Sarcopenia, is associated in cohort studies with falls, frailty, slower recovery from illness and higher all-cause mortality, and low muscle combined with high fat appears worse than either alone. These are associations from observational data, not proof that changing one number changes the outcome.
What changes it
- Resistance training. The most reliable lever. Randomized trials in adults of all ages, including people in their eighties, show increased muscle mass and strength from two or more sessions a week.
- Protein intake. Older adults generally need more than the classic minimum; roughly 1.2-1.6 g per kg of body weight a day is the range most often recommended for preserving muscle, spread across meals.
- Energy balance. A calorie deficit reduces fat mass, but fast weight loss without training and adequate protein takes lean mass with it. The same caution applies to weight-loss medication.
- Sleep and alcohol. Short sleep and heavy drinking both work against muscle retention and favor abdominal fat.
- Outside your control. Age, sex, genetics and hormonal change, such as the shift of fat toward the abdomen after menopause, set the terrain you train on.
