Fat-free mass (FFM)

Fat-free mass (FFM) is everything your body weighs once fat is subtracted: skeletal muscle, bone, organs, connective tissue, blood and the water held inside all of it. Body weight is only two things, fat mass and fat-free mass; FFM is all of the second half. It is often used interchangeably with lean body mass, though lean body mass strictly also counts the essential fat inside cell membranes, marrow and nerve tissue. Body-composition reports use Fat-free mass (FFM) for the tissue that does the metabolic work.

What it measures

No device weighs fat-free tissue directly. Every method estimates fat, or a proxy for lean tissue, and reaches FFM by subtraction or formula. A DEXA scan separates bone mineral, fat and lean soft tissue with low-dose X-rays and is the practical reference standard. Bioelectrical impedance devices, including smart scales, estimate body water from electrical resistance and convert it using an assumed hydration fraction of lean tissue – convenient, but sensitive to what you have drunk, eaten and sweated. Air displacement and underwater weighing work from whole-body density; skinfold equations depend on who takes them.

FFM is not muscle. It includes bone, organs and fluid, and skeletal muscle is only around half of it in a healthy adult. When the question is specifically about muscle, the more precise term is Skeletal muscle mass (SMM), which body-composition devices report separately.

Typical values

Because FFM is weight minus fat, its typical value follows directly from body fat percentage: a person at 20% body fat is 80% fat-free. Men carry a higher fat-free share than women at the same fitness level, and the number rises with height and frame, so raw kilograms tell you little alone.

To compare people, FFM is normalized by height squared, giving FFMI — fat-free mass index in kg/m2, the lean-tissue counterpart of BMI. Commonly quoted adult averages sit in the high teens for men and the mid teens for women, but published reference ranges differ by population, age and device, so read your result against the range your own scan supplies. Clinical thresholds for low muscle mass are not set on FFM at all: they use appendicular muscle indexes, and cut-offs differ between the European and Asian working group definitions and between DEXA and impedance.

Why it matters for longevity

Fat-free mass burns most of your energy at rest, stores most of the glucose from a meal, and is the reserve you spend during illness. In observational studies, low lean mass is associated with frailty, falls, longer hospital stays, worse recovery from surgery and higher all-cause mortality, and low lean mass with high fat mass appears worse than either alone.

It also matters during weight loss. Any calorie deficit takes some lean tissue along with fat, and trials of rapid weight loss, including with newer weight-loss drugs, report that a meaningful share of what is lost is fat-free. These are associations, not proof that raising one number on a scan changes how long you live.

What changes it

  • Resistance training. The most reliable lever, and it still works in the eighties. Randomized trials show gains from two or more sessions a week.
  • Protein. Older adults need more than the classic minimum; roughly 1.2-1.6 g per kg of body weight a day is the range most often recommended for preserving lean tissue, spread across meals.
  • How you lose weight. A slower deficit with training and enough protein preserves far more fat-free mass than a crash diet.
  • Creatine. Reliably raises measured FFM, partly through real gains and partly by pulling water into muscle cells, which inflates the reading.
  • Hydration and timing. On impedance devices, a large drink or a hard workout can move your reading more than months of real change, so measure in the same state each time.
  • Outside your control. Age, sex, height, genetics and hormonal shifts set the baseline; slow loss of lean tissue from midlife is the default.
