Homocysteine

Homocysteine is a sulfur-containing amino acid the body produces while processing methionine, an amino acid from dietary protein, and its level in the blood climbs when the B vitamins needed to clear it run short. You do not eat it and do not need it: it is a way station meant to be cleared quickly, and when that traffic backs up it accumulates. That is why the blood level is read as a marker of B vitamin status and vascular risk. What it measures

The test is an ordinary blood draw, reported as total homocysteine (tHcy) in micromoles per liter and usually taken fasting. Sample handling matters: red cells keep releasing homocysteine after the draw, so a tube left standing before it is spun reads falsely high.

What the number reflects is how well two disposal routes are working. The first is remethylation back to methionine, which needs Folate (vitamin B9) as the methyl donor and vitamin B12 as the cofactor of the enzyme that transfers it. The second is transsulfuration, which converts homocysteine toward cysteine and depends on vitamin B6. The kidneys handle a large share of clearance, so reduced kidney function raises the level independently of vitamin status.

Laboratories differ, but the commonly published bands look like this:

Treat those edges as conventions, not physiology. The interval depends on the assay, and levels run higher in men, after menopause, with age and with reduced kidney function. Population averages differ by country too, because mandatory folic acid fortification of flour lowered them wherever it was introduced. Some clinicians prefer a target nearer 10, but that is a judgement call, not an agreed standard.

Why it matters for longevity

The observational evidence is large and consistent. Higher homocysteine is associated with cardiovascular disease and stroke, with faster cognitive decline, brain atrophy and dementia, with fractures, and with all-cause mortality. Plausible mechanisms exist: injury to vessel linings, promotion of clotting, and interference with methylation reactions the brain depends on.

The trials have been more sobering. Lowering homocysteine with folic acid and B vitamins did not reduce heart attacks in the large cardiovascular prevention trials. Some analyses found a modest reduction in stroke, most visible where flour is not fortified, and genetic studies point the same way, toward stroke more than coronary disease. A randomized trial in people with mild cognitive impairment and raised homocysteine reported slower brain shrinkage on B vitamins; trials in unselected older adults have been null. The honest reading: homocysteine is a reliable signal that something is wrong, and a weaker lever than its associations suggest.

What changes it

Find out why it is high rather than pushing it down blindly. Test vitamin B12 and folate, check kidney function and thyroid, and review medications: metformin and long-term acid suppression lower B12, and methotrexate, some anticonvulsants and nitrous oxide interfere with the same pathways. Correcting a confirmed deficiency is where the reliable benefit sits.

Folate comes from leafy greens, legumes, beets and citrus; B12 comes only from animal foods or supplements, which makes it the standing gap on a vegan diet and in older age, when absorption declines. Heavy drinking and smoking push the level up. Genetics contribute too: the enzyme variant described under MTHFR — the gene and the C677T polymorphism makes the folate route less efficient, so carriers of two copies tend to run higher when folate intake is low. One caution: high-dose folic acid taken without checking B12 can mask a B12 deficiency while the nerve damage continues, so test before you supplement.
