What caloric restriction is

Caloric restriction (CR) is the most reproducible life-extending intervention in animals, but no study has yet shown that it extends lifespan in humans. The strongest human evidence comes from the CALERIE trial, in which healthy adults cut their energy intake by about 12% for two years and improved their cardiometabolic markers. Those are the same signals captured by everyday indices of insulin sensitivity such as METS-IR: metabolic score for insulin resistance, which is worked out from glucose, triglycerides, HDL and BMI. CR means a sustained reduction in energy intake without malnutrition, so every nutrient you need is still covered. It is not fasting and not an extreme diet held at the edge of tolerance, but a moderately leaner way of eating sustained over the long term.

What the animal data show

In yeast, worms, flies and rodents, caloric restriction has extended lifespan and delayed age-related disease consistently for decades. It is one of the most reliable findings in the biology of aging. In primates the picture is more complicated: two large studies in rhesus macaques gave mixed results for lifespan itself, but health improved in both, with better metabolism, better cardiovascular measures, and less diabetes and cancer.

What caloric restriction does in humans

The key trial is CALERIE: healthy adults without obesity moderately restricted calories for about two years, by roughly 12% on average. The results:

  • cardiometabolic markers improved, including blood pressure, lipids, insulin sensitivity and inflammation;
  • a separate analysis found a slower pace of aging on the epigenetic marker DunedinPACE — the clock of the pace of aging;
  • safety was monitored throughout, and the thing to avoid is sliding into a deficit of protein, vitamins or minerals.

There is still no direct evidence that caloric restriction extends lifespan in humans specifically, and a trial able to prove it is close to impossible to run. But an improvement in health markers is already a meaningful result. Caloric restriction is not about being hungry all the time. Chronic undereating, muscle loss and nutrient deficiencies are harmful and work against longevity.

Caloric restriction, fasting and everyday practice

Many people find it easier not to count calories every day and to use a simpler frame instead, such as intermittent fasting, which often lowers intake on its own. The practical minimum for most people: do not overeat, build meals around whole foods, keep protein high enough, and do not lose Lean muscle mass. Who should not restrict calories

Caloric restriction is not appropriate if you are underweight, in adolescence, pregnant or breastfeeding, have a history of an eating disorder, live with certain chronic conditions, or are older and at risk of Sarcopenia. In those cases the goal is the opposite: enough food and enough protein.
