Mild cognitive impairment (MCI) is a measurable decline in memory or thinking that is greater than expected for a person’s age and education, but not severe enough to take away everyday independence. It is the middle ground between normal cognitive aging and dementia. Amnestic MCI is memory-led; in non-amnestic MCI, attention, language, planning or visuospatial skill slips first. MCI names a stage, not a cause: early Alzheimer’s pathology, small-vessel disease, thyroid disorder, B12 deficiency, sleep apnea, depression and sedating medication all produce the same test results. The subtypes and reversible causes gathered under Mild cognitive impairment (MCI) matter, because the label alone says nothing about what happens next.
What it measures
The diagnosis rests on three findings: someone has noticed a change, testing confirms it objectively, and daily life still runs without help. Testing ranges from a short clinic instrument such as the MoCA or MMSE to a full neuropsychological battery, with scores read against norms for age, education and language rather than one universal pass mark. Blood work and imaging mainly look for treatable contributors: thyroid function, B12 and folate status, medication, alcohol, sleep apnea, low mood. Amyloid and tau markers can point to Alzheimer’s disease as the cause, but they do not define the syndrome: MCI describes function, not pathology.
Typical values
There is no blood value for MCI. What can be graded is where someone sits between normal aging and Dementia, on testing and in real tasks.

Screening cut-offs are a starting point, not a verdict. The MoCA was originally validated with a threshold below 26 out of 30, but that line behaves differently by education, language and test version, so clinics adjust it rather than diagnose from one score.
Why it matters for longevity
MCI is the most useful early signal that a brain is under strain, and it arrives while the causes can still be treated. Its course is genuinely uncertain: in specialist memory clinics a substantial share progress to dementia each year, in community samples the rate is lower, and a meaningful minority return to normal cognition — often those whose cause was sleep, mood, medication or metabolism and was then corrected. Where amyloid and tau markers are positive, progression is more likely. MCI is also associated in cohort studies with falls, medication errors, driving difficulty and higher mortality, partly because it travels with the same vascular risk factors.
What changes it
Age and inherited risk are not levers. The reversible contributors are, and they come first: correcting B12 deficiency or an underactive thyroid, treating sleep apnea and depression, cutting alcohol, reviewing sedating drugs, and correcting hearing and vision loss. Vascular control is the best-evidenced lever after that: in the SPRINT MIND trial, intensive blood pressure lowering reduced new cases of mild cognitive impairment, though the effect on probable dementia alone was not statistically significant. Multidomain programs of exercise, a Mediterranean-style diet, cognitive training and vascular risk management have modestly improved test scores in at-risk older adults. Education, demanding work and lifelong mental activity build Cognitive reserve, associated with later symptom onset for the same underlying damage. No supplement and no brain-training app has been shown to prevent progression. Anti-amyloid drugs, licensed in some countries for MCI caused by Alzheimer’s disease, slow decline somewhat, carry a real risk of brain swelling and bleeding, and reverse nothing. If a change in memory or thinking is new, persistent and noticed by others, get it assessed rather than watched.
