Gestational diabetes is high blood glucose first detected during pregnancy in a woman not previously known to have diabetes. It develops because placental hormones make tissues less responsive to insulin and the pancreas cannot always raise output enough to keep up. It rarely causes symptoms, so it is found by a scheduled test rather than by how a woman feels, and glucose usually normalizes within weeks of delivery. What does not go away is the metabolic risk the pregnancy exposed.
What it measures
The diagnosis rests on an oral glucose tolerance test: a measured glucose drink, then blood draws at set intervals to see how fast you clear it. Testing is normally done at 24 to 28 weeks, when Insulin resistance peaks in pregnancy; women with risk factors are tested earlier, mainly to catch diabetes that was already present. Two protocols exist. The one-step approach gives a 75 g load after an overnight fast and samples at zero, one and two hours, and a single value at or above threshold is enough to diagnose. The two-step approach starts with a non-fasting 50 g screening drink and a one-hour draw; only if that is high does a 100 g three-hour test follow, where two or more values must reach threshold. HbA1c is not used for the diagnosis, because red-cell turnover and blood volume both shift in pregnancy.

Those are the widely adopted IADPSG and World Health Organization cut-offs, and one value reaching any of them is enough. The two-step route uses a different set: the commonly used Carpenter-Coustan criteria for the 100 g three-hour test are 95 mg/dL fasting, 180 mg/dL at one hour, 155 mg/dL at two hours and 140 mg/dL at three hours (5.3, 10.0, 8.6 and 7.8 mmol/L), with at least two values required; older criteria set the bar higher still. Why it matters for longevity
Within the pregnancy, higher maternal glucose is associated with a larger baby, more cesarean deliveries, preeclampsia and low blood sugar in the newborn, and treating it reduces those outcomes in randomized trials. The longer-range signal matters more for healthspan. Follow-up cohorts report a large increase in later Type 2 diabetes among women with this history — commonly cited as close to a ten-fold relative risk, though the absolute figures vary widely with follow-up length, diagnostic criteria and population. Observational data also link it with higher later cardiovascular risk and hypertension. Read it as an early stress test: pregnancy loaded the metabolic system and it did not cope — information most women would otherwise not have for decades. What changes it
During pregnancy, most cases are managed first with diet and movement — spreading carbohydrate across the day, favoring intact grains, legumes and vegetables over refined starch and sugary drinks, and walking after meals — alongside home glucose monitoring. When targets are not met, insulin is the usual next step, and metformin is used in some settings; those decisions belong with the maternity team. After delivery the most valuable action is testing: a glucose tolerance test a few weeks postpartum to confirm glucose has normalized, then repeat testing every one to three years for life. Breastfeeding is associated with lower later diabetes risk in the mother. Weight management, strength and aerobic training, and adequate sleep all act on the same underlying insulin resistance. Some risk factors cannot be changed: age, family history, ethnic background, polycystic ovary syndrome and gestational diabetes in a previous pregnancy.
