Vascular age

Vascular age is the state of your arteries expressed as an age: the chronological age at which the average person’s blood vessels look the way yours look now. It converts a measurement of arterial stiffness, plaque or cardiovascular risk into years, so a 50-year-old can be told his arteries resemble a typical 62-year-old’s. What it measures

Vascular age is never measured directly. It is a translation of another reading into years, so its meaning depends on which reading was used.

The most common physical input is pulse wave velocity. Sensors record how fast the pressure wave from a heartbeat travels between the carotid artery in the neck and the femoral artery in the groin. A stiff vessel carries that wave faster than an elastic one, so the speed reads Arterial elasticity directly. Comparing it with age-specific population values gives the age at which that speed would be average.

Other inputs behave differently. A coronary artery calcium score from a low-dose CT counts calcified plaque. Heart-age calculators such as Framingham and QRISK use no vessel measurement at all: they take your blood pressure, lipids, smoking and diabetes status and report the age of a person with the same predicted risk but ideal risk factors. Function tests such as Flow-mediated dilation (%FMD) read how well a vessel widens on demand. Same word, four different meanings.

Typical values

A result is given as a vascular age in years plus the gap to your chronological age. There is no agreed cut-off for how large that gap must be to count, and a few years either way sits inside the noise of most methods.

Reference values differ by device, formula, sex and population, so a vascular age from one clinic does not transfer to another. Compare only with an earlier result from the same method.

Why it matters for longevity

The underlying measurements have a solid observational record. Higher aortic pulse wave velocity has been associated with more cardiovascular events and higher all-cause mortality in pooled cohort analyses, independently of standard risk factors. A calcium score of zero has been associated with low event rates in large screening cohorts. Stiff arteries also load the heart, brain and kidney, which is why stiffening tracks with heart failure and cognitive decline, not only heart attack.

The age label adds no biology. It is a presentation format: “your arteries look 12 years older than you are” lands harder than a wave speed in meters per second. No trial has shown that moving the figure by itself changes outcomes; what changes outcomes is treating the blood pressure, lipids and smoking behind it.

What changes it

Blood pressure is the main lever, and lowering it has improved pulse wave velocity in trials. Aerobic exercise improves arterial compliance, most in people who were sedentary. Cutting sodium, losing weight, controlling blood sugar and stopping smoking move stiffness the same way.

Plaque-based numbers behave differently. Calcium scores rarely fall, and statin treatment can leave a score flat or higher while still lowering risk, because plaque becomes denser and more stable. A vascular age built on calcium is a risk statement to read once, not a scoreboard to track. Chronological age remains the largest driver of arterial stiffening, and that part does not reverse.
