Systolic pressure

Systolic pressure is the higher of the two blood pressure numbers: the peak force blood exerts against the artery walls while the heart contracts and pushes blood out. In a reading of 132/78 mmHg, the systolic pressure is 132 millimeters of mercury. It reflects how forcefully the left ventricle ejects blood and how well the large arteries absorb that surge. In most populations it keeps climbing with age while the lower number flattens or falls after mid-life, which is why clinicians watch it most closely in older adults.

What it measures

A cuff on the upper arm inflates until it halts flow in the brachial artery, then deflates. The pressure at which blood first pushes through again is the systolic value; a clinician hears it as the first tapping sound, an automatic monitor infers it from cuff oscillations. The number is a snapshot of one arm at one moment, and it moves with posture, talking, caffeine, cold and stress.

Technique matters as much as the device. Sit for about five minutes with your back supported and feet flat, keep a correctly sized cuff at heart level, stay quiet, and average two or three readings a minute apart. Because a clinic visit itself raises the number in many people, guidelines increasingly rely on home averages over several days or on 24-hour ambulatory monitoring. Subtracting the lower number gives Pulse pressure, a related figure that widens as the aorta stiffens.

These cut-offs are not universal. European guidance has long used 140 mmHg as the systolic threshold for diagnosing hypertension, so an identical reading can be labeled differently depending on which guideline your clinician follows. Home and daytime ambulatory averages are judged against lower thresholds than office readings, because they leave out the clinic effect. Why it matters for longevity

Systolic pressure is one of the best-quantified risk markers in medicine. In large pooled analyses of observational cohorts, the risk of death from stroke and heart disease rises continuously with systolic pressure, with no clear point below roughly 115 mmHg where lower stops being better. Past about age 50 the systolic number predicts cardiovascular events more strongly than the diastolic one, largely because a high systolic reading with a normal or low diastolic reading signals stiff large arteries rather than a hard-working heart. That pattern, isolated systolic hypertension, is the most common form of high blood pressure in later life, and it tracks closely with loss of Arterial elasticity.

This marker also has randomized evidence behind it, which most aging markers lack. Trials of blood-pressure lowering have reduced strokes, heart attacks and heart failure, and in high-risk adults a more intensive systolic target reduced cardiovascular events and all-cause mortality. Sustained elevation is also associated in cohort data with kidney decline and later cognitive impairment.

What changes it

Several levers genuinely move the number. Losing excess weight lowers systolic pressure, and so does cutting sodium, eating more potassium-rich plants in a DASH-style or Mediterranean pattern, limiting alcohol, and training regularly, with aerobic, isometric and resistance work all lowering it in trials. Short sleep and untreated sleep apnea push it up. When readings stay high despite these, medication is the main lever, and several drug classes lower systolic pressure effectively; the choice is a clinical one. Some drivers are not negotiable: arteries stiffen with age, family history counts, and kidney or hormonal disease can raise pressure secondarily and needs diagnosis rather than lifestyle work alone.
