Alkaline phosphatase (ALP)

Alkaline phosphatase (ALP) is an enzyme that strips phosphate groups off other molecules, and the amount measured in blood comes mainly from two places: the lining of the bile ducts in the liver and the bone-building cells of the skeleton. It appears on almost every standard metabolic and liver panel. On its own a raised result says only that something is happening in the liver, the bile ducts or bone — not which, and not what. Because it straddles two organ systems, Alkaline phosphatase (ALP) is always read next to the other enzymes on the same panel.

What it measures

The value comes from a routine blood draw and is reported in units per liter (U/L). Fasting is not usually required, though a fatty meal can briefly lift the intestinal form of the enzyme in some people. What the lab prints is total ALP: the sum of several isoenzymes made by the liver, bone, intestine and, during pregnancy, the placenta. In a non-pregnant adult, the liver and bone forms account for almost all of it.

Finding the source is the whole job. The practical shortcut is a second enzyme that rises with bile-duct trouble but not with bone turnover: a high ALP together with a raised GGT (gamma-glutamyl transferase) points toward the liver and bile ducts, while a high ALP with a normal GGT points toward bone. When the picture stays unclear, a laboratory can fractionate the isoenzymes directly.

Why it matters for longevity

ALP is one of the nine blood markers, combined with chronological age, that make up PhenoAge, a phenotypic age score built from a routine panel to predict mortality better than birth date alone. In large population cohorts, higher ALP is associated with higher all-cause and cardiovascular mortality, and that association shows up across the upper part of what labs still call normal, not only in obvious liver disease. This is observational evidence: ALP marks what is happening, it has not been shown to cause it.

One proposed mechanism is that the enzyme breaks down inorganic pyrophosphate, a natural brake on mineral deposition in artery walls, so more activity could mean less braking and more vascular calcification. That is a plausible biological link, not a settled one in humans. The everyday value is simpler: a cheap early flag for cholestatic liver disease and for silent bone disorders.

What changes it

Nothing you do moves ALP directly. It moves when its cause does, so the number is a reason to look, not a target to chase.

On the liver side, cutting alcohol and losing excess weight lower it when fatty liver is the driver. Many medications can cause a cholestatic pattern, and the enzyme settles after the drug is changed, which is a decision for the prescriber, not a self-experiment. Gallstones and other bile-duct obstruction raise it until they are treated.

On the bone side, anything that speeds bone turnover raises it: a healing fracture for some months, adolescent growth, an overactive parathyroid gland, Paget disease of bone. Long-standing severe deficiency of Vitamin D raises the bone form by causing osteomalacia, and correcting the deficiency brings it back down. Drugs that slow bone turnover, such as bisphosphonates, lower it. A low result is read the other way around: zinc and magnesium are cofactors for the enzyme, so poor status can depress it.
