Albumin

Albumin is the most abundant protein in blood plasma, made by the liver, and it both holds fluid inside the blood vessels and ferries fatty acids, calcium, bilirubin, hormones and many drugs around the body. It sits on almost every routine blood panel because it reports on three things at once: how well the liver is synthesizing, whether protein is being lost or under-supplied, and how much inflammation is running. A low value is one of the most common abnormal results in sick and older adults, and it is one of the blood markers fed into modern biological-age scores.

What it measures

Serum albumin comes from an ordinary venous draw, as part of a comprehensive metabolic or liver panel, and is reported in g/dL in the US and g/L elsewhere. The result is a concentration, so plasma volume moves it without changing how much albumin your liver makes: dehydration and a tourniquet held too long push it up, intravenous fluids, pregnancy and the plasma expansion of heavy endurance training pull it down. Albumin turns over slowly – its half-life in health is roughly three weeks – so it lags behind whatever is happening today. Subtract it from total protein and the rest is the globulin fraction. Do not read those numbers as universal. The two dye methods in common use, bromocresol green and bromocresol purple, do not return identical values on the same sample, so the interval printed on your own report is the one that applies. Ranges also shift with age and pregnancy, and children have intervals of their own. The trend across several draws tells you more than one value sitting near a cut-off.

Why it matters for longevity

Albumin is a negative acute-phase protein: when inflammation starts, the liver diverts synthesis toward acute-phase proteins such as C-reactive protein (CRP), capillary walls leak, and albumin falls. That makes it a rough inverse gauge of inflammatory load. In observational cohorts of older adults, lower serum albumin is associated with higher all-cause mortality, more frailty and sarcopenia, longer hospital stays and worse outcomes after surgery, and the association holds for values still inside the normal band. It is also one of the nine blood markers combined with chronological age in PhenoAge — phenotypic age, which is why a modest fall shows up as an older estimated age. The honest reading is that albumin mostly marks illness rather than causing it: raising the number with an infusion does not buy the outcomes, outside specific hospital indications.

What changes it

The lever is nearly always the underlying cause, not the protein. Treat the infection, calm the chronic inflammation, cut alcohol if the liver is the problem, and address kidney or bowel losses, and albumin follows on a timescale of weeks. Eating more protein raises it only when intake was genuinely inadequate; in a well-fed adult extra protein does little to serum albumin, though it still matters for muscle. Before you act on a borderline result, rule out the boring explanations: dehydration, posture and a long tourniquet can each shift it enough to cross a line. Some of it is not yours to move: a gradual decline is part of normal aging.
